Health Office

Two nurses are on staff at Saint Dominic Elementary School Health Office to answer questions, administer medication (with written permission) and help insure that the stu­dent body remains healthy.

Special Notices

Students who are entering 6th grade on or after September 1, 2007, and who are 11 years of age or older must receive an immunization containing tetanus toxoids, diphthe­ria, and a cellular pertussis (Tdap).

Any student who arrives late to school should stop at the Health Office to let the nurse know he or she is in school.

Medications

As per the New York State education department, a school nurse is not permitted to ad­minister any medication (including Tylenol) unless written doctor and parental permission is on file. The permission slip should include frequency and dosage of medications, the condition for which it is prescribed, and a completed authorization form from the parent/guardian. Forms are available in the Health Office.

Please be reminded that state regulations mandate that students are not permitted to carry medication on them while in school unless they have written doctor and parental permission on file. All medication (prescription or non-prescription) is to be brought to the school nurse by an adult. In addition, cough drops are not administered to students by the Health Office.

Sick Child Guidelines

If your child has vomited or had a fever since dinner or needs Tylenol in the morning to be able to come to school, he/she needs to stay home another 24 hours. Otherwise, their germs can be spread to and infect other students. Please remember to phone the health office if your child is staying home due to illness.

Physical Exams and Screenings

In accordance with New York State Law, physical examinations are required for all students in kindergarten and grades 2 and 4 or students new to the district.

 

Emergency Information Card

Emergency information cards on file in the school nurse’s office have parents’ home and work telephone numbers, doctors’ names and numbers and persons to be contacted in emergencies if parents cannot be reached. Emergency infor­mation cards are also required in order for a child to leave the building on a field trip. It is vitally important that parents keep the nurse’s office apprised of any changes in address, telephone numbers, places of employment, and persons to be contacted in case of emergency.
